Dementia Care Coordination is a partnership program between the Alzheimer’s Association & a Health System designed to change the medical management of Alzheimer’s disease and related dementias.
The Key to DCC is an outgoing call from a trained clinician at the Alzheimer’s Association to the caregiver.
The goal of the outgoing call is to provide the caregiver resources and strategies before a crisis situation occurs.
DCC was piloted in 2013; since then, we have partnered with fourteen Health Systems to provide support to over 4000 families
DCC is a cost-sharing model in which philanthropic dollars fund the program initially; the Health System is able to collect outcomes and utilization data to support funding for the program after the initial period. There is no charge to the patient or the caregiver for any of our services
